Medical coding

Offshore medical coding staff for medical practices

OffshoreMeds places offshore medical coding staff with medical practices: full-time coders who assign ICD-10, CPT and HCC codes in your EHR and practice management system. Senior coding leads audit their work daily against the 95% accuracy rate the Journal of AHIMA calls the de facto standard for medical coding. We confirm each candidate’s credentials before you interview them. One flat monthly fee per seat, never a percentage of collections. Clinical decisions stay with your US-licensed providers.

The work

What offshore medical coders do

  • ICD-10 and CPT coding from your providers’ notes, in your EHR and practice management system.
  • HCC and risk adjustment coding for practices with risk-adjusted contracts.
  • Audits that sample finished work and report what was wrong and why.
  • Provider queries. When a note does not support a code, the coder asks your provider instead of guessing. Clinical decisions stay with your US-licensed providers.

Quality

How coding accuracy is measured

An accuracy rate is the share of audited codes that are correct. The Journal of AHIMA (Stanfill, January 2019) describes 95% as the de facto standard for medical coding. At that standard, an audit of 100 codes allows no more than 5 errors.

Ask any vendor how it counts accuracy: by code, by claim or by chart. When accuracy is counted per claim, one wrong code makes the whole claim count as wrong, so the same work can score lower per claim than per code.

Grid of 100 squares standing for audited codes: 95 are filled dark to show codes that must be correct at the 95% accuracy standard, and 5 are light to show the most errors the standard allows.

Oversight

Daily audits and credential checks

  • Senior coding leads audit coders’ work daily against the published standard.
  • We confirm each candidate’s credentials before you interview them.
  • You interview, test and approve each hire. Nothing starts without your sign-off.
  • Coders work in your systems under your access controls, with your coding policies.

A practice can also run its own audit. The sampling guidance, the credential lookups and the warning about reading a Medicare error rate as a coder accuracy rate are in how to audit medical coding accuracy.

Revenue impact

Coding accuracy and claim denials

Wrong codes are one source of claim denials. MGMA Stat reports first-submission denials of 7-8% over the past four years. Root-cause analysis by denial management staff shows whether your denials trace back to codes, to eligibility or to a missing prior authorization.

Cost

What a coder's seat costs

A coder’s seat is priced like any other seat: $1,500 to $2,000 per month. Prices are indicative until your written quote confirms the exact fee. See the full offshore medical billing cost comparison against a US in-house hire, and the HIPAA rules for offshore work.
